The wellness360™ Concept

Q: How is this possible–how can wellness360 really pay me to be a member without some sort of “catch”?

A: It is hard to believe, but it's true--just by using the features and services on the wellness360 website, you'll earn points that translate into dollars. There is no cost to join, and you don't have be part of a corporate program, or meet some sort of purchase requirements.

The concept is simple: wellness360’s revenues come from advertisers who pay for the opportunity to deliver highly focused messages to our wellness360 members. Everyone wins–advertisers reach the appropriate people, the web pages you view contain only content that may be relevant to you, and we return a good portion of what we are paid to those who are actively staying informed and healthy (YOU)!

Q: I’ll be providing some personal information on the wellness360 site--how do I know that my privacy is protected?

A: We comply fully with the government-mandated policy, HIPAA (Health Insurance Portability and Accountability Act) which protects the privacy of your medical information. You control access to your information at all times. We’re also compliant with the stringent statutes of the COMIA (California Confidentiality of Medical Information Act), in addition to providing a one million dollar privacy guarantee!!

Aperture Health constantly takes steps to ensure compliance with HIPAA well in advance of the compliance dates for covered entities. That is why wellness360 is designed with administrative, technical and physical safeguards in order to maintain the integrity and confidentiality of our members' data. These safeguards, as well as all Aperture Health policies, are also continually reviewed analyzed and updated as part of Aperture Health's ongoing commitment to protect the privacy of our members' data and to comply with HIPAA and all applicable laws.

Point Earning and Redeeming Cash

Q: How do I redeem points for cash?

A: Below the “My Points” chart, showing your monthly totals, found here , you will notice a “Redeem Points” button. This button is active only during the first fifteen (15) days after the end of each calendar quarter—and only whenever your point value equals or exceeds $25 for that previous quarter. Your points will be zeroed-out once you’ve redeemed them. If you miss a redemption period, your points WILL continue to accumulate until the next available period.

Q: When are the quarters?

A: Financial quarters run: Jan.-Mar., Apr.-June, July-Sept. and Oct.-Dec.

Q: When will the Redeem Points button be active again?

A: The first fifteen days of the end of each quarter means: April 1-15, July 1-15, Oct. 1-15, and Jan. 1-15.

Q: How and when will I get paid?

A: Your earned cash will be sent to your street mail address in the form of a personal check. Checks are mailed 5-6 weeks after the end of each quarter.

Q: What activities on the site earn the most points?

A: The actual points earned per wellness360 page access, or activity, is not fixed. The amount of points you can earn will be highest if you make good use of the features and services offered on the site. The cash translation per point also isn't a fixed and provided value, but there is always a fair and close correlation between points and dollars, and it's always the case that the more you use the wellness360 website, the more points you'll earn for that period.

Periodically, specific site features are promoted as “high points” pages, meaning that using them will allow you to periodically earn points at a higher rate. (See the wide “Bonus Points!” button above “My Points” on the right side of the screen after you’ve logged in.)

Some specific activities that will always earn points on the wellness360 website include:
? Taking our Health Risk Assessment* and following its recommendations
? Tracking what you eat
? Recording your daily exercise
? Reading articles that interest you
? Updating and uploading your personal health records
*Your Health Risk Assessment must be completed before points are redeemed.

Q: How will you know that I didn’t just click on everything, put in bogus data, or even design a program to do so?

A: The wellness360 software is designed to catch fraudulent usage. In order to discourage inappropriate activity, the specific number of points assigned for various page accesses, etc., is not released to the general public.

Q: What do you do if inappropriate use of the site is detected?

A: Aperture Health reserves the right to withhold points if it has been determined that the data entered is meaningless. If an excessive and not typical amount of pages accesses are detected, your points will be deducted or even zeroed-out, your member account may be deactivated, and you can ultimately be permanently prohibited from applying for membership in the future.

Invite a Friend Program

Q: How do I get paid for friends who join?

A: Simply stated, you get paid when they get paid. You will not see their points accruing as part of your points total; you just will receive 25% of the cash that they are paid. In specific cases where we’ve offered extra points for inviting a friend, however, those points will be added to your personal total within a few days of the promotion.

When any of your referred friends uses the Redeem Points button (active only during the first fifteen (15) days after the end of each calendar quarter—and only whenever their point value equals or exceeds $25), they will be sent their earned cash in the form of a personal check, and YOUR check will contain an additional 25% of that value.

As with your account, if they miss a redemption period, their points will continue to accumulate until the next available period, and you will not receive your 25% of what they’ve earned until they redeem those points.

wellness360™ Site Features/Problems

Q: When I try to login, why does it keep telling me that the “challenge question” isn’t answered correctly?

A: Before you retry, you must close your browser window and start fresh with a new login screen, rather than just editing the values that you’d already entered before.

Q: I still can’t login. What do I do now?

A: Fill out the “Contact Us” form found here, provide the Member ID you are attempting to use, and be sure to include your email address and full name. Someone will contact you with a valid ID and password.

Q: Why do I need a Personal Health Record?

A: Doctors have expressed that anytime a physician is going to make an important clinical treatment decision for you, having the latest and most accurate information in your Personal Health Record (PHR) becomes critical. By filling out your Personal Health Record (PHR), you will be taking charge of your personal medical information and possibly having an impact on future diagnoses and treatment advice from your doctor(s).

If you change doctors, medical plans, or if you move, you’ll also be able to save time by having copies of your medical information (your PHR) conveniently available for your new physician. The average HMO patient changes primary care physician at least once a year, and few realize that it is the patient’s responsibility to ensure that their complete health record is transferred to their new doctor(s). By consolidating all of your medical information in one location and under your control, you can be assured that your records will be complete, up-to-date, and accessible to you, your doctor (if desired) or emergency personnel at any time, any place, and anywhere you have an Internet connection.

Q: I’ve been getting an error when taking my Health Risk Assessment (HRA), at about question #40. What should I do?

A: The HRA has been modified to allow you to start over if you encounter a system error. To avoid one common error, make sure that you don’t answer gender-specific questions that don’t apply to the gender you’d already identified for yourself earlier in the survey.

Q: I’m not clear about the use of the Food Intake & Exercise Tool, are there some basic guidelines that may help?

A: The uniqueness of the Food Intake & Exercise Tool is that you view all of your food and exercise plans (or actual foods eaten/exercise completed) for a particular date, on one simple screen. To change to a different date, just click on the desired date on the calendar provided on the upper left of the screen.

Q: Can I refer to food and exercise that I selected for previous days?

A: Yes, when you’re on the Food Intake & Exercise Tool page for a particular day, just scroll down to the “Copy food (or exercise) content over from a previous day” section(s) and select a date from the pull-down menu provided. At this time, you will have to delete any specific items from the copied list that you do not want to include for the date you are working on.

Note to our seasoned wellness360 members: To improve the overall functionality and value of our food and exercise tracking functions, the prior “My Refrigerator”, “My Workout”, “Food Journal” and “Calendar” features have been replaced by the new, powerful-yet-simplified Food Intake & Exercise Tool.
